When a patient needs ongoing care, subacute care steps in. Joint Commission accredited, Community Subacute & Transitional Care Center is one of the largest facilities in California with a 106-bed facility providing inpatient care for chronic, subacute conditions such as coma or other complex illness that require 24-hour medical supervision.
Patients whose care requires ventilator, respiratory, wound, IV, hospice, rehabilitation services and/or general nursing can receive treatment that’s consistent with an acute care center’s effectiveness while in a comfortable long-term care setting.
