In a busy, demanding hospital environment, oversees, coordinates, and communicates all care and services provided for patients from admission through discharge. Serves as family and patient contact regarding any care or length of stay issues. Provides concurrent and ongoing review of patient charts based on screening criteria developed and approved by the medical staff. Prepares reports based on information obtained from the chart reviews, assists assigned departments of the Medical Staff in implementing effective quality and utilization management systems. This is the first of two levels of Case Manager RN. EXPERIENCE AND EDUCATION:
MINIMUM REQUIRED: Graduation from an accredited School of Nursing or successful completion of sufficient units in Nursing to qualify for state licensure. One year experience in acute care and working knowledge of acute care process/procedure, medical terminology, and general medical diagnoses and diagnostic procedures.
PREFERRED: Licensed Registered Nurse with a variety of experiences in a hospital environment caring for patients and their families.
MINIMUM REQUIRED: Current California Registered Nurse License. Current BLS certification.
