Community Regional Medical Center (CRMC) is adding an additional position to its existing Case Management team in the Emergency Department.
Community Regional Medical Center is seeking nurses like you for challenging 12 hour shifts!
In this role you will work in close partnership with Social Workers, Discharge Navigators and the Interdisciplinary clinical team. RN Case Managers gather information from a wide variety of resources to create, implement and deliver safe discharge plans for our patient population. They use evidence based software for review and streamlined technology for discharge patients.
Minimum requirements include: Bachelor’s degree in Nursing and one year acute care hospital experience. Four years of acute Case Management experience may be substituted in lieu of education. Current RN license for the state of California and BLS certification.
