Responsibilities

The Patient Safety Coordinator supports Community Health Partners (CHP) in improving patient safety, quality outcomes, and regulatory compliance by coordinating event reporting, investigation, including support of patient complaints and grievance processes, analysis, and performance improvement activities. The role maintains patient safety programs and initiatives that provide a safe environment for patients, visitors, and personnel. 
 
The coordinator serves as a key resource for clinical and operational teams, providing education and data analysis to identify opportunities for improvement and reduce patient safety risks. This position collaborates across departments, including Community Health System partners, to implement evidence-based improvements, monitor safety and quality trends, and support improvement initiatives. Through strong partnership with leadership and frontline teams, the Patient Safety Coordinator promotes a culture of safety and continuous improvement.
 
 
 

Qualifications

Education
High School Diploma, High School Equivalency (HSE), or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
 
Experience
3 years of experience in a healthcare environment required
Demonstrated clinical or operational experience working with cross-functional teams required
Experience supporting quality, patient safety, or performance improvement initiatives required
