Responsibilities

The RN Case Manager is responsible for providing support and clinical expertise across the continuum of care, from admission through discharge or transition to the next level of care. Collaborates with physicians and the interdisciplinary team to coordinate patient care, enhance clinical outcomes, support patient and family engagement, and manage the cost and efficiency of care delivery. This role integrates utilization management, care coordination, and discharge or transition planning functions. Through close collaboration with the care team, the RN Case Manager ensures that appropriate services are delivered in a timely manner and aligned with patient needs, goals, and regulatory guidelines.

Qualifications

Education

  • High School Diploma, High School Equivalency (HSE), or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required

 

Experience

  • 1 year of nursing experience in the assigned care setting (e.g., acute care, home health, skilled nursing) required

  • Prior case management experience preferred

 

Licenses and Certifications

  • RN - Current and valid Registered Nurse license to work within the state of California required

  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
