Responsibilities

Located on the Community Regional campus, Burnett Extended Care Center is our brand-new 154-bed transitional care facility. Purpose-built to serve patients transitioning from hospital to home, Burnett includes specialized units — Memory Care, Long-Term Care, and Transitional Care — that focus on quality, comfort and continuity of care. This environment offers nurses the opportunity to provide patient-centered care across diverse needs in a collaborative, post-acute setting. 

  • Provide direct nursing care to patients recovering from illness, surgery, or hospitalization, many with complex medical or rehabilitative needs. 
  • Collaborate with an interdisciplinary team to develop and implement individualized care plans that support patient goals and safe transitions of care. 
  • Monitor and assess patient conditions, administer medications, and document care in accordance with established policies and standards. 
  • Foster a supportive, team-oriented environment that promotes collaboration, open communication, and continuous learning.

Qualifications

Education
• High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
• Bachelor's Degree in Nursing preferred

 

Experience
• Consistent demonstration of Competent clinical nursing practice proficiency required

 

Licenses and Certifications
• RN - Current and valid Registered Nurse license to work within the state of California required
• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
