H.M. Providers, Inc. dba Community Hospitalist Physician Group in Fresno, CA is seeking a full-time hospitalist.
In this role you will provide hospital-based inpatient medical services consistent with best practices, evidence-based medicine, the Joint Commission core measures, and appropriate resource utilization strategies. Provide professional consultative expertise in general internal medicine consistent with best practices, evidence-based medicine, the Joint Commission core measures, and appropriate resource utilization strategies. Participate in the daily multidisciplinary rounds with case management to review each inpatient's clinical, psychosocial, and disposition status. Diagnose, treat, and provide continuous care to hospital inpatients. Actively evaluate patients in the Emergency Department (ED) in a timely manner, and collaborate with the ED physicians to avoid unnecessary admissions, including prescribing medication and treatment regimens. Engage in regular communication with referring primary care physician and consultants regarding each inpatient's clinical, psychosocial, and disposition status, including notifying all relevant physicians upon admission, discharge, or change in clinical status. Ensure timely documentation of all patient encounters, including dictated history and physical, consultations, and discharge summaries within 48 hours of encounter. Refer patients to specialists or other professionals as needed. Document inpatients’ records using EPIC. Participate in appropriate divisional, departmental, and institutional committees and activities. Assist with ongoing data collection, data analysis, and performance improvement initiatives. Ensure compliance with state and federal laws, accreditation, and professional and regulatory agency standards, including quality assurance and licensure requirements. Maintain a teamwork mentality to ensure optimal patient care and comprehensive sign-outs and hand-offs from one physician to another.
Salary: $329,309.85 to $357,390.20 per year
*American Board of Internal Medicine (ABIM) certificate or eligibility, as proven by: 36-month residency, Doctor of Medicine degree or equivalent, Educational Commission for Foreign Medical Graduates certificate, or The National Board of Physicians and Surgeons certificate or eligibility, and demonstrated clinical competence in overall clinical competence in each year of training by receiving satisfactory ratings in each of the ACGME/ABMS Competencies during the final year of required training; or by letter issued by ABIM confirming that ABIM’s training requirements for the Certification Examination in Internal Medicine have been fulfilled. The six ACGME/ABMS Competencies are: (1) patient care and procedural skills, (2) medical knowledge, (3) practice-based learning and improvement, (4) interpersonal and communication skills, (5) professionalism and (6) systems-based practice.
