Located on the Community Regional campus, Burnett Extended Care Center is our brand-new 154-bed transitional care facility. Purpose-built to serve patients transitioning from hospital to home, Burnett includes specialized units that focus on quality, comfort, and continuity of care. 

• Oversee the planning, implementation and evaluation of activities that encourage engagement and enhance quality of life for residents. 

• Participate in Interdisciplinary Team (IDT)’s meetings, committee meetings and care plan conferences. 

• Plan and conduct staff in-service training as needed, fostering a collaborative and innovative environment. 

• Ensure facility residents have an activity plan in place and the participation of that plan is documented. 

By consistently following the resident regimen and activity plan, you help uphold a standard of excellence that defines the Burnett Care Center experience throughout every stage of care. 

Education and Experience 

• High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required 

• One of the following is required 

• Two years of experience in a social or recreational program within the last five years, one year of which was full-time in a patient activities program in a health care setting. 

• Be an occupational therapist, art therapist, music therapist, dance therapist, recreation therapist, or occupational therapy assistant. 

• Have satisfactorily completed at least 36 hours of training in a course designed specifically for this position and approved by the facility and shall receive regular consultation from an occupational therapist, occupational therapy assistant or recreation therapist who has at least one year of experience in a health care setting.
