Responsibilities

Located on the Community Regional campus, Burnett Extended Care Center is our brand-new 154-bed transitional care facility. Purpose-built to serve patients transitioning from hospital to home, Burnett Care Center includes specialized units that focus on quality, comfort, and continuity of care. 

  • Oversee the daily operations of the Social Services department to ensure residents and families receive timely psychosocial-emotional support. 
  • Assist residents and families in navigating the emotional and psychosocial challenges related to illness, disability or recovery. 
  • Promote a supportive and compassionate environment through advocacy, communication and individualized care planning. 
  • Oversees the completion of Social Services minimum data set (MDS) assessments, care plans and required documentation. 
  • Mentor peers and healthcare staff on issues related to social services. 

 

As a social services leader, you’ll be the compassionate bridge between clinical care and emotional well-being — advocating for residents, guiding families and ensuring every transition is handled with the dignity, empathy and excellence expected at Burnett Care Center. 

Qualifications

Education and Experience 

  • Bachelor's Degree in Social Work (BSW) required 
  • 3 years of relevant experience in the healthcare industry or healthcare setting required 

 

Licenses and Certifications 

  • LCSW - State of California Licensed Clinical Social Worker preferred
