Responsibilities

As a RN Home Health Coordinatory, you are responsible for the clinical review and assisting with the prioritization of visits for all patient referrals.  Assists with the coordination of all aspects of patient care with home health clinicians, physicians, and any other patient care/service providers.  Provides clinical support to the workflows in Community Home Care to ensure quality patient care.

Qualifications

EXPERIENCE AND EDUCATION:
Graduation from an accredited School of Nursing or successful completion of sufficient units in Nursing to qualify for state licensure. One year of clinical experience as a Registered Nurse.

PREFERRED: One year of home health experience.

 


LICENSES/CERTIFICATIONS
Current California Registered Nurse license. Current BLS certification. Current California Driver License and driving record acceptable to Community Medical Centers.
