In this role you will work collaboratively with the Case Management staff and other health care team members to coordinate and execute discharge planning services for patients through the healthcare continuum. You will help assist in the execution of a plan of care to facilitate a safe discharge in a timely manner and serves as a liaison on community facilities and resources. With a patient ratio of 1:45 your day is anything but average.
Education
• One of the following is required
o Bachelor's Degree in Social Work or related field
o Completion of an accredited Vocational Nursing School program
Experience
• 1 year of experience in Case Management with prior assessment and referral experience required
Licenses and Certifications
• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
• LVN - Current State of California Licensed Vocational Nurse License for incumbents without a qualifying Bachelor's Degree required
• Pay ranges listed are an estimate and subject to change.
