Responsibilities

We are looking for LVN’s to join our skilled nursing teams at CSTCC! As an LVN on our dynamic team, you will care for a variety of including short-term and long-term rehabilitation patients. You will be integral to patient care, helping ensure continuity of care of the residents between shifts, as well as supervising the care given by CNAs and supporting staff members. 

 

When a patient needs ongoing care, subacute care steps in. Community Subacute & Transitional Care Center is a 106-bed facility providing inpatient care for chronic, subacute conditions such as coma or other complex illness that require 24-hour medical supervision. 

 

Patients whose care requires ventilator, respiratory, wound, IV, hospice, rehabilitation services and/or general nursing can receive treatment that’s consistent with an acute care center’s effectiveness while in a comfortable long-term care setting.

Qualifications

  • High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
  • Completion of an accredited Vocational Nursing School program OR become licensed by equivalency required
  • 6 months of experience in a long term care facility preferred
  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
  • LVN - Current State of California Licensed Vocational Nurse License required
