RN, Clinic Coordinator, Neuro Institute

*All positions are located in Fresno/Clovis CA*

The RN Clinic Coordinator CHP provides professional telephone triage nursing services to Community Health Partners (CHP) patients. Utilizes the nursing process based on research and evidence-based outcomes to meet patient care needs. Collaborates with other members of the health care team to ensure the delivery of safe, timely, appropriate, quality patient care. Facilitates appropriate utilization of healthcare resources, provides education and various patient and physician care coordination services.

Education
• Graduation from a school of Nursing or successful completion of sufficient units in nursing qualifying for state licensure required

Experience
• 3 years of progressive responsibility for patient care required

Licenses and Certifications
• RN - Current State of California Registered Nurse license required
• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
