Community Health Partners is seeking a full-time Pharmacist, specializing in oncology in the outpatient setting of Community Cancer Institute. Community Cancer Institute is Central California’s premier comprehensive cancer care center. The Community Health System (Community Medical Centers "CMC") is a locally owned, not-for-profit, public-benefit organization based in Fresno, California. Community is the region's largest healthcare provider and private employer. We operate a physician residency program with one of the nation's top medical schools – the University of California, San Francisco. Community is home to the only Level 1 Trauma Center and comprehensive burn center between Los Angeles and Sacramento. We also serve as the area's "safety net provider." In fiscal year 2020, Community provided nearly $175 million in uncompensated services and programs. In addition to our hospitals, our health system includes a vast medical network, medical foundation, medical group and health plan. The Community Provider Network is a network of provider affiliates and partners that provides our patients access to over 1,400 physicians. Community Health Partners and Community Physicians Group are our newly formed medical foundation and multi-specialty medical group. Our health plan, Community Care Health, is the third largest HMO in the region. We also operate four acute-care hospitals – Community Regional Medical Center, Clovis Community Medical Center, Fresno Heart & Surgical Hospital and Community Behavioral Health Center – as well as several long-term care, outpatient and other healthcare facilities. This position requires a commitment to excellence in patient care, collaboration, communication, teaching, research and a dedication to place the patient at the center of all decisions. Clinical or translational research interests are encouraged and supported.
 

Accountabilities

 

  1. Enter, adjust, and/or modify orders and dosages based on guidelines on provider’s behalf
  2. Troubleshoot treatment and therapy plan issues received from Oncology physician team
  3. Collaborate with providers during drug shortages and make proper modifications to treatments
  4. Review, implement, and assist in the creation of clinical trial protocols
  5. Coordinate with in-patient team on proper transitions of care
  6. Create treatment plan protocols based on provider’s request
  7. Consult patients on medications (chemotherapy, oral medications, injections, etc.)
  8. Answer clinical questions for physicians, APPs, medical assistants, and staff
  9. Review all treatment plans and therapy plans for accuracy and completion
  10. Utilize reports to review, renew, and discontinue treatment plans on a weekly basis
  11. Optimize Electronic Medical Record System use and improve workflow for all providers
  12. Participate in the Oncology Pharmacy and Therapeutics team as well as the Beacon Workgroup team in protocol review and implementation
  13. Precept pharmacy students
  14. Resolve billing issues on provider’s behalf

Qualifications

Licenses and Certifications

  • A current license to practice as a Registered Pharmacist issued by the California State Board of Pharmacy (Required)
  • A current license to practice as an Advanced Practice Pharmacist issued by the California State Board of Pharmacy (Preferred)
  • At least two years’ experience in oncology (Preferred)
