Community Regional Medical Center is the flagship hospital of Community Medical Centers – a private, not-for-profit healthcare system based in Fresno, California, and the largest healthcare provider and private employer in central California. Assists patients who present with complex psychosocial needs, and works collaboratively with the Case Management team, health care providers, and physicians. Intervenes to help the patient and/or family understand, and cope with personal, emotional and bio-psychosocial issues affecting hospitalization and the delivery of care. Interventions include comprehensive assessment and referral, crisis intervention, and case management. Collaborates within a multi-disciplinary team, and community agencies to facilitate a safe discharge plan through the continuum of care.?In addition, the Social Worker - ED possesses the skills to assess patients presenting with mental health issues, and collaborates with the physician to determine the most appropriate post discharge care. This position promotes individual professional growth and development by providing direct supervision of of MSWs with less than 2 years of hospital experience and MSW graduate interns.
Master's degree in Social Work (MSW). Two or more years of acute hospital social work experience. Experience in psychosocial and therapeutic assessments and interventions. LICENSES/CERTIFICATIONS MINIMUM REQUIRED: Current CA. Licensed Clinical Social Worker (LCSW). Fresno County Department of Behavioral Health 5150 certification must be obtained within 6 months of hire into position or obtaining LCSW licensure. Current BLS certification. PREFERRED: Critical Incident Stress Management certification (CISM). Accredited Case Management certification (ACM).
