    As a key member of our Rehab team you will further our mission by:

    • Serving as a liaison between Community Medical Centers and the community by initiating, intervening, and evaluating nursing services to patients at CMC.

    In this role some of your key responsibilities would include but are not limited to:


    • Performs physical therapy screening in a technically competent manner. Identifies signs and symptoms that signal appropriateness for PT examination. 
    • Examines and evaluates the patient's abilities and functional limitationS
    • Maintains a varied case load of medically complex patients including oncology major multi trauma, brain injury and spinal cord injury. 
    • Establishes plan of treatment and goals based on the examination and evaluation of selected test and measures.
    • Implements therapy according to established plan, utilizing appropriate procedures, equipment and techniques within the scope of physical therapy.
    • Provides ongoing physiologic and functional assessment of patient and communicates changes in patient’s condition to healthcare team, including calling physician as needed.
    • Demonstrates knowledge of medical and post-surgical precautions and adjusts examination/treatment accordingly.
    • Demonstrates knowledge of laboratory and diagnostic tests/results and responds appropriately. Ability to recognize and react quickly to changes in patient’s condition. Demonstrates awareness of emergency situations and performs duties appropriately.
    • Consults and collaborates with other hospital staff; discuss patient progress, set objectives for patient treatment and integrate physical treatment with other inpatient care.
    • Participates in developing discharge plan and recommendation of next level of care for patient.(e.g. in-pt rehab, subacute placement, SNF, OP Rehab, Home Health Care, etc.). Evaluates home environment and makes physical barrier modifications and equipment recommendations.
    • Educates patient, family / caregiver, and other hospital staff about the findings of the examination and evaluation, established goals of the physical therapy program, plan of treatment, home programs, injury prevention, and other related information.
    • Assists in directing PT Assistants with their caseloads and their documentation.
    • Assists in directing and/or coordinating student clinical educational experience.
    • Produces documentation that is complete, accurate, concise, timely and legible and meets all requirements according to according to Community Medical Centers standards.
    • Provides an environment conducive to safety for patients, visitors, and staff. 


    Rehab services are provided across the continuum, including a Neuroscience center and Primary Stroke Center, Joint Replacement Center of Excellence, Level III NICU, and the Leon S. Peters Rehabilitation Center.


    Our staff is a balanced mix of seasoned clinicians, recent graduates and everything in between. This offers an optimal environment for mentoring and implementation of the most current evidenced based practice.


    We support regular continuing education to maintain currency and further develop our staff. Our therapists regularly share knowledge through mentoring and in-services.


    Experience and education minimum requirements:

    • Bachelor's degree in Physical Therapy.
    • State of California Physical Therapy license.
    • Current BLS from American Heart Association or American Red Cross only accepted.
    • One year of prior PT experience.
