    Responsibilities

    Works collaboratively with Case Management staff and other health care team members to assist in the execution of a plan of care to facilitate a safe discharge in a timely manner.

    Qualifications

    Associates degree in Social Work or completion of an LVN program and two years' experience in Case Management/assessment and referral.

    Bachelor's degree in Social Work or Healthcare/Health Sciences related field may be substituted in lieu of education and experience.

     

    PREFERRED: Bachelor's degree in Social Work or healthcare related field.

     

    LICENSES/CERTIFICATIONS MINIMUM REQUIRED: Current CA LVN license for incumbents without qualifying degree. Current BLS certification.
