    The Risk Management Specialist Patient Safety  position is a vital member of our mission.


    In this position you will be responsible for managing tasks to provide risk management and patient safety support including; analyzing and documenting clinical/reporting processes, developing and documenting action plans to reduce risk and improve quality of care, and participating in system planning and quality/patient safety meetings. Provides a linkage to internal and external resources/services to manage all clinical patient care cases in which an adverse event has occurred. Works with confidential information on a daily basis, works closely with other Analysts and Risk Managers in patient safety and risk mitigation including presentation of Quality/Safety Improvement measurement activities at Community Medical Centers.

    • Supports a facility "culture of safety" through the development of a transparent and data-driven, proactive, preventative facility Patient Safety Program. Reviews and updates program annually. Serves as a facility liaison for patient safety culture surveys. Responsible for promotion of survey at the facility and POS regarding culture of safety. 
    • Oversees the management and use of patient error/occurrence information: reviews internal error reports and utilizes information from external reporting programs such as ISMP Medication Safety, ECRI Health Device Alerts and NQF data bases. Supports and encourages error reporting throughout the organization through a "just culture" error reporting system.
    • Performs in-depth qualitative analysis for identified high risk patient safety incidents.
    • Responds to CDPH surveyor investigations and coordinates staff involvement and plan of correction response if required. Maintains a CDPH log and reports any adverse events to Corporate Risk Management/Patient Safety Officer for follow-up and reporting to CDPH.
    • Coordinates and directs the root cause analysis for critical clinical adverse events.
    • Provides consultative assistance 24/7 on-call to leadership, physician and staff regarding high risk situations with potential to impact patient safety and corporate liability. Communicates and assists members of the healthcare team in the documentation of facts in the medical record and in communicating and disclosing with patients and families following unanticipated medical healthcare events.
    • Assists the hospital medical malpractice carrier and defense counsel during the litigation process by answering interrogatories, obtaining appropriate information when requested for production of documents and by representing the hospital at mandatory settlement conferences, mediations, depositions, staff interviews and trials. Provides expertise ensuring effective defense of the hospital.
    • Provides education to facility leadership, staff, new RN grads, and residents related to risk prevention and reporting of medical healthcare errors. Provides on-going education based on clinical issues identified from adverse events, patient safety goals, just culture, and through the discovery process of litigation.
    • Reviews all unanticipated events to determine referral to medical malpractice carrier. Works with carrier in the aggregation and analysis of potential claims data to identify opportunities for improvement as well as providing expertise to attempt settlement of cases with potential litigation prior to involvement of defense counsel in an effort to decrease risk related expense to the corporation.
    • Assists in the resolution of serious patient/family grievances in collaboration with customer service and medical malpractice carrier.
    • Participates as a member of multiple corporate committees involved in the development of standards, practice changes, and improvement of systems/processes to ensure patient safety. Acts as a liaison between the facility leadership and the facility Quality Patient Safety Committee.
    • Recommends and facilitates change to improve patient safety based on identified risk assessment. Participates in facility and system-wide safety initiatives. Collaborates in the development of facility policies/procedures effecting patient safety. Develops and implements an internal communication system educating and communicating patient safety alerts. Participates and coordinates in service line safety teams including responsibility for tracking and referral to Quality Management for PDSA team assignment.
    • Provides instruction and training in methods and procedures to Risk Management staff. Plan, assign and approve completion of work duties and/or projects to ensure accuracy and efficiency.


    Minimum required:

    Master’s degree in Statistics, Business, or Healthcare related field and 5 years experience in Quality/Risk and Patient Safety fields. Previous employment in a highly confidential position.



    Minimum required:

    National certification in Risk Management and/or Quality Management (CPHQ) or Certified Professional in Healthcare Risk Management (CPHRM) within 2 years of hire.
